(the email address you enter for yourself will be copied in the email as well).

If you have trouble viewing or submitting the form using your browser, or if you do not have an email client (such as Outlook) on your computer be sure to  download the file and save it to your computer first, then complete the form, save it and use your email program to send it to bcampbell@datasis.com as an attachment. 

Please use Acrobat or the Acrobat Reader to view/complete the form (some third-party pdf viewers including Preview (Mac) are not compatible with Acrobat forms).  Also, Some versions of Chrome and Firefox have their own pdf viewers that will not open Acrobat forms properly:  https://get.adobe.com/reader/
